  The story I heard was that there was a local pizza restaurant near Hollywood that was pretty popular and did deliveries. And for some reason they included anchovies on their "supreme" (or "combination" or whatever you call it) pizza by default. So local screenwriters who didn't like anchovies had to get in the habit of adding "...and hold the anchovies," on every order. And then the writers stuck that in their scripts—characters specifying they didn't want anchovies on their pizzas, regardless of who they were ordering from—either as an inside joke, or because they keep forgetting that the rest of the world isn't like LA. And thus anchovies became a punchline.
