It doesn't cost extra resources for someone to use design talent + willingness to let anyone in the game die. It's just that developers don't want to do it anymore because of "triple A" standards. As for realism: Skyrim is a game where you can shoot fire and lightning out of your hands while battling dragons.
Oh, but it does.
Letting anyone in the game die while still leaving the plot and major subplots intact (which Bethesda's really finicky about) means adding a lot more dialogue, redundancy in the cast, coding to allow branching paths to even exist. And that means more time at the office, more time running the electricity, more lunches stolen from the marketing team's refrigerator. And that means more bills to pay, which means more money to spend, which means less profit for the investors and eventually the people who actually made the game.
The "being able to kill everybody is realistic" argument is weird to me, because it assumes that everybody always wants to kill everybody else.
Anyway, I have a big decision coming up in this EU4 game, whether to convert to Protestantism, convert to Reformed Christianity, or to remain Catholic.
Furthermore, realism in the setting is not the same as verisimilitude. A world may break the laws of physics as we know it and still feel like a "real" world with actual consequences and characters who act like real people would in that kind of world. The reverse is also true: Consider the average Harlequin romance.

Yes, but the Elder Scrolls games attempt to create verisimilitude through extensive, consistent worldbuilding, through lots of choices for the player, and through high realism with regards to aspects not directly related to magic or fantasy creatures (the society, the environment, that kind of thing). There is no particular canonical reason why, in Skyrim, the Dragonborn would never under any circumstances kill Maven, nor why she should be immortal.
The suggestion that realism is irrelevant in a fantasy is one that irritates me. You often see people attempting to justify inconsistent characterization and plotting in fantasy novels on the same grounds. Fantasy doesn't automatically imply nonsense, and I feel that to equate the two is to disparage the genre.
