Last night I dreamed I was exploring the junior high school I went to. Foxit Reader 5.1 seems to have done undone a bit of the UI uglification that 5.0 introduced.
It still doesn't use any of the native window toolbars or anything, but neither did version 4. But it uses the native window chrome now and that's good enough for me.

1961 -- note the outlined US shields, and the use of "THRU TRAFFIC" rather than a route designation:
1971, 1978, 1988 (all used the same illustrations)
2000 -- for the first time, these were distributed as vector graphics in a PDF file. Note also that the text-only "US 56" sign is still included, despite most states having already discontinued this practice decades ago.
2003: The proper typefaces were used, but the signs still aren't quite pattern-accurate. (Note the inline fraction for "1/2", for example)

...what leads to new MUTCD editions?
I'm not sure there's anything specific that leads to new editions. New editions usually include changes based on studies about how to make signage more efficient/effective. For instance, the latest edition forbids the use of names (road names, places names, any names) in all-caps, since studies have shown mixed-case is more legible. It also forbids the use of signs like the one on the right here, because studies showed that drivers mistakenly assumed "exit only" applied to both lanes.
I wonder why signs are green
In the ye olde days of the 1940s and 1950s, guide signs were primarily black and white--smaller signs, like you'd see on conventional roads, where black-on-white, and overhead signs like you see on freeways were white-on-black. Then Caltrans (the California Department of Transportation) experimented with a few different color combinations, and found that white on green was the most legible.
