First order: the plot would have to be toned down. This is simply because an SNES cartridge is not built for all that sheer plot. The basics can be kept, sure, but all the complex shit (like Doc Scratch/Lord English's plan) is a no-go. I mean, seriously. There would probably be a vague reference here and there (like "A higher power commissioned Jack for mysterious purposes" or "John, you are destined to create yourself", but only that.

Second of all, it would be infested with SNES tropes/cliches common at that time. John would have to be completely silent, Derse and Prospit would require a revamp to reflect the "bad planet/good planet" dynamic even more, etc

Thirdly, the music would need to be more atmospheric. There would probably be less techno-bloopy things or sweet guitar licks (if there were, this would be a Genesis game) and more shit like this. 

Hivebent would need to be a sequel. Again, an SNES cartridge is not built for that kind of shit.
