    My guesses, based upon a geology professor I once knew who was from Indiana (who, when called out on his bad pronunciations of local towns, would ask people to pronounce towns from Indiana and then laugh when people messed up).

    "Cai" like the letter k, "ro", like the first word of "row, row, row your boat".
    "Green" like the color, "wich" like the last four letters of "sandwich".
    "Hous" like the name of a dwelling (house), "ton" so as to rhyme with "run".
    "Lima" like the bean.
    "Mil" like the last four letters of "windmill", and "an" like the first two letters of "anchovy".
    "helphelpIamastupideasternerwhocannotpronouncethingsproperlypleaseforgivemeandtakemymoney".
    "Rio" like the first syllable of Scooby Doo attempting to say "Dio Brando", "Grande" rhymes with "candy".
    "Ver" like the first three letters of "Vermont", "sailles" like the word "sails".

    okay, i guess i should answer seriously too

    Cairo - see note below, except I wouldn't have guessed "cay-row"
    Greenwich - I know the English place name is pronounced "Grennich" and so I am figuring that this is all unusual place name pronunciations so it's probably "GREEN-witch"
    Houston - the first syllable is almost certainly pronounced like "house" or "how's", and I think I've heard of this before
    Lima - this probably is pronounced the way English-speakers say "lima bean", with a long i, rather than the Spanish pronunciation (even though that's also the source of the bean name).
    Milan - assuming unusual pronunciations, i would guess "MY-lun" (or "MILE-un"), but 
    Nevada - I know of people dinging others for pronouncing the second syllable as a short a vs. an "ah" a.  I forget which one is the official pronunciation of the state name, but presumably the Ohio location is the other one.
    Rio Grande - I'd guess that the second word is pronounced like "grand" in English rather than "grande" in Spanish.
    Versailles - I'd guess that this is pronounced like "ver-SAIL", or better yet, "ver-SAILS", in contrast to its French pronunciation.

    It's funny, because the place is called "Rio Grande, Ohio" but "Rio Grande" and "Ohio" both just mean "big river"
