Breaking up the sign borders:

"EXIT ONLY" patch too short:

Inappropriate use of mixed case; also "2nd" on a road sign is dumb:

This one may actually be the City of Delaware's doing; I don't think ODOT maintains signs like this anymore:

"BOTH LANES" patch wider than the actual sign:

Patch over a patch--yes, the original text (the yellow you see sticking out) did say "EXIT ONLY":
